ആലപ്പുഴ: ആലപ്പുഴ മെഡിക്കല്‍ കോളേജ് ആശുപത്രിയില്‍ ശസ്ത്രക്രിയയ്ക്കിടെ യുവതിയുടെ വയറ്റില്‍ കത്രിക (ഫോഴ്സ്പ്സ്) വച്ചു മറന്ന സംഭവത്തില്‍ ശസ്ത്രക്രിയ നടത്തിയ ഡോക്ടര്‍ ഷാഹിദയ്ക്ക് അന്വേഷണ റിപ്പോര്‍ട്ട് ക്ലീന്‍ ചിറ്റ് നല്‍കി. ഇതേസമയം, സംഭവത്തില്‍ കൂടെയുണ്ടായിരുന്ന നഴ്‌സ് കുറ്റക്കാരിയാണെന്നു വരുത്തിത്തീര്‍ക്കാനും ശ്രമം. 

ശസ്ത്രക്രിയയ്ക്കിടെ പാലിക്കേണ്ട എല്ലാ മെഡിക്കല്‍ മാനദണ്ഡങ്ങളും ഡോക്ടര്‍ ഷാഹിദ കൃത്യമായി പാലിച്ചിരുന്നുവെന്നും അവര്‍ക്ക് വ്യക്തിപരമായ വീഴ്ചകള്‍ സംഭവിച്ചിട്ടില്ലെന്നും റിപ്പോര്‍ട്ട് വ്യക്തമാക്കുന്നു.

ശസ്ത്രക്രിയയ്ക്ക് മുന്‍പും ശേഷവും ഉപകരണങ്ങള്‍ എണ്ണിത്തിട്ടപ്പെടുത്തേണ്ട ചുമതലയുള്ള നഴ്സിനും മറ്റ് അനുബന്ധ ജീവനക്കാര്‍ക്കുമാണ് വീഴ്ച സംഭവിച്ചതെന്ന് റിപ്പോര്‍ട്ടില്‍ പറയുന്നു. എല്ലാ ഉപകരണങ്ങളും കൈവശമുണ്ടോ എന്നു ഡോക്ടര്‍ ചോദിച്ചിരുന്നുവെന്നും ഉവ്വെന്നു നഴ്‌സ് ധന്യ മറുപടി കൊടുത്തുവെന്നുമാണ് റിപ്പോര്‍ട്ടില്‍ പറയുന്നത്. ധന്യയ്ക്കു തൊട്ടടുത്ത ടേബിളില്‍ നടന്ന ശസ്ത്രക്രിയയിലും സഹായിക്കേണ്ടതുണ്ടായിരുന്നുവെന്നും റിപ്പോര്‍ട്ടില്‍ പറയുന്നു.

കോവിഡ് കാലത്താണ് ഈ ശസ്ത്രക്രിയ നടന്നത്. അക്കാലത്ത് ആശുപത്രികളില്‍ ഉണ്ടായിരുന്ന ജീവനക്കാരുടെ കുറവും അമിത ജോലിഭാരവും ഇത്തരം ഒരു പിഴവ് സംഭവിക്കാന്‍ കാരണമായിട്ടുണ്ടാകാമെന്ന് റിപ്പോര്‍ട്ട് ചൂണ്ടിക്കാണിക്കുന്നു.

ഇതൊരു ബോധപൂര്‍വ്വമായ വീഴ്ചയല്ലെന്നും സാഹചര്യങ്ങള്‍ കൊണ്ട് സംഭവിച്ചുപോയ ഒറ്റപ്പെട്ട കാര്യമാണെന്നുമാണ് അന്വേഷണ സംഘത്തിന്റെ വിലയിരുത്തല്‍. 2021 മേയ് 12-ന് ആലപ്പുഴ വണ്ടാനം മെഡിക്കല്‍ കോളേജിലാണ് പുന്നപ്ര സ്വദേശിനി ഉഷാ ജോസഫിന്റെ ശസ്ത്രക്രിയ നടന്നത്. ഗര്‍ഭാശയത്തിലെ മുഴ നീക്കം ചെയ്യാനായിരുന്നു ശസ്ത്രക്രിയ.

ശസ്ത്രക്രിയയ്ക്ക് ശേഷം കഴിഞ്ഞ വര്‍ഷമായി ഉഷയ്ക്ക് കഠിനമായ വയറുവേദനയും ശാരീരിക അസ്വസ്ഥതകളും ഉണ്ടായിരുന്നു. പലതവണ വണ്ടാനം മെഡിക്കല്‍ കോളേജില്‍ ചികിത്സ തേടിയെങ്കിലും മൂത്രത്തില്‍ കല്ലാണ് വേദനയ്ക്ക് കാരണമെന്നായിരുന്നു ഡോക്ടര്‍മാര്‍ പറഞ്ഞിരുന്നത്. എന്നാല്‍ അടുത്തിടെ അസ്വസ്ഥതകള്‍ കൂടിയപ്പോള്‍ നടത്തിയ എക്‌സ്‌റേ പരിശോധനയിലാണ് വയറ്റില്‍ ശസ്ത്രക്രിയയ്ക്ക് ഉപയോഗിക്കുന്ന കത്രിക കുടുങ്ങിക്കിടക്കുന്നത് കണ്ടെത്തിയത്.

ഡോ. ലളിതാംബിക

അന്ന് ശസ്ത്രക്രിയയ്ക്ക് നേതൃത്വം നല്‍കിയ ഗൈനക്കോളജി വിഭാഗം മേധാവിയായിരുന്ന ഡോ. ലളിതാംബികയ്ക്കും സംഘത്തിനുമെതിരെയാണ് കുടുംബം പരാതി ഉന്നയിച്ചത്. എന്നാല്‍ ശസ്ത്രക്രിയ ചെയ്തത് താനല്ലെന്നും ആ സമയത്ത് താന്‍ റിട്ടയര്‍മെന്റിന്റെ വക്കിലായിരുന്നതിനാല്‍ ഭരണപരമായ കാര്യങ്ങളിലായിരുന്നു ശ്രദ്ധയെന്നും ഡോ. ലളിതാംബിക വിശദീകരിച്ചു. സിസ്റ്റത്തിന്റെ പിഴവാണിതെന്നും ഉപകരണങ്ങള്‍ എണ്ണിത്തിട്ടപ്പെടുത്തുന്നതില്‍ നഴ്‌സുമാര്‍ക്ക് വീഴ്ച പറ്റിയതാകാമെന്നും അവര്‍ പറഞ്ഞു.

സംഭവം വിവാദമായതോടെ ആരോഗ്യ മന്ത്രി വീണാ ജോര്‍ജ് അന്വേഷണത്തിന് ഉത്തരവിട്ടു. വിദഗ്ധ സമിതി നടത്തിയ അന്വേഷണത്തില്‍ ഡോക്ടര്‍ക്ക് നേരിട്ട് വീഴ്ച സംഭവിച്ചിട്ടില്ലെന്നും എന്നാല്‍ ഇന്‍സ്ട്രുമെന്റ് കൗണ്ട് പരിശോധിക്കുന്നതില്‍ നഴ്‌സുമാരുടെ ഭാഗത്ത് വീഴ്ച ഉണ്ടായതായും റിപ്പോര്‍ട്ട് നല്‍കി. ഇതിന്റെ അടിസ്ഥാനത്തില്‍ ഡോക്ടര്‍ക്ക് ക്ലീന്‍ ചിറ്റ് നല്‍കുകയും നഴ്‌സുമാര്‍ക്കും മറ്റ് സ്റ്റാഫിനുമെതിരെ നടപടിക്ക് ശുപാര്‍ശ ചെയ്യുകയും ചെയ്തു.

മെഡിക്കല്‍ കോളേജില്‍ വീണ്ടും ശസ്ത്രക്രിയ നടത്താന്‍ ഉഷയ്ക്കും കുടുംബത്തിനും പേടിയായതിനെ തുടര്‍ന്ന്, കെ.സി. വേണുഗോപാല്‍ എം.പിയുടെ ഇടപെടലിലൂടെ ഇവരെ കൊച്ചിയിലെ സ്വകാര്യ ആശുപത്രിയിലേക്ക് മാറ്റി. അവിടെ വച്ച് വിജയകരമായ ശസ്ത്രക്രിയയിലൂടെ വയറ്റിലുണ്ടായിരുന്ന കത്രിക പുറത്തെടുത്തു. ഈ ഉപകരണം നിലവില്‍ പോലീസ് തൊണ്ടിമുതലായി കസ്റ്റഡിയില്‍ എടുത്തിട്ടുണ്ട്.

'ശരീരത്തില്‍ ഇത്തരം ഉപകരണം 50 വര്‍ഷം ഇരുന്നാലും കുഴപ്പമില്ല' എന്ന ഡോ. ലളിതാംബികയുടെ പ്രസ്താവന വലിയ രീതിയിലുള്ള പൊതുജന പ്രതിഷേധത്തിന് കാരണമായിരുന്നു. നിലവില്‍ ഈ സംഭവത്തില്‍ പോലീസ് അന്വേഷണവും വകുപ്പുതല നടപടികളും പുരോഗമിക്കുകയാണ്.

പുതിയ റിപ്പോര്‍ട്ട് വരുന്നതോടെ ഡോക്ടര്‍ക്കെതിരെയുള്ള നടപടികള്‍ ഒഴിവാകാനാണ് സാധ്യത.

ഇത്തരം വീഴ്ചകള്‍ ഗൗരവകരമാണെങ്കിലും ചില പ്രത്യേക സാഹചര്യങ്ങളില്‍ സംഭവിക്കുന്ന ഒറ്റപ്പെട്ട കാര്യങ്ങളായി ഇതിനെ കാണണമെന്നാണ് സിപിഎം സംസ്ഥാന സെക്രട്ടറി എം.വി. ഗോവിന്ദന്‍ പറയുന്നത്.

The report highlights that the shortage of staff and the extreme workload prevalent in hospitals at that time might have contributed to such an oversight. The investigation team concluded that this was not a willful act of negligence but an isolated incident caused by difficult circumstances.

Background of the Case

The surgery on Usha Joseph, a native of Punnapra, took place on May 12, 2021, at Alappuzha Vandanam Medical College to remove a uterine tumor. Following the surgery, Usha suffered from severe abdominal pain and physical discomfort for years. Although she sought treatment at the same hospital multiple times, doctors repeatedly told her the pain was due to kidney stones. However, as the discomfort intensified recently, an X-ray was taken, revealing the surgical forceps lodged in her abdomen.

The family initially filed a complaint against Dr. Lalithambika, the then-head of the Gynecology department, and her team. However, Dr. Lalithambika explained that she did not perform the surgery herself as she was nearing retirement and was focused on administrative duties. she attributed the error to a "systemic failure" and lapses by the nursing staff in counting instruments.

As the incident sparked controversy, Health Minister Veena George ordered an investigation. The expert committee’s report concluded that the doctor was not directly at fault, but there was a failure on the part of the nurses in verifying the instrument count. Based on this, the doctor was given a clean chit, while disciplinary action was recommended against the nurses and other staff.

Fearing another surgery at the Medical College, Usha and her family sought help, and following the intervention of K.C. Venugopal MP, she was moved to a private hospital in Kochi. The forceps were successfully removed through surgery and are currently in police custody as evidence.

Dr. Lalithambika’s previous statement—"It doesn't matter even if such instruments remain in the body for 50 years"—had triggered massive public outrage. Currently, both a police investigation and departmental proceedings are ongoing.

With the release of this new report, legal actions against the doctor are likely to be dropped. Meanwhile, CPM State Secretary M.V. Govindan remarked that while such lapses are serious, they should be viewed as isolated incidents occurring under specific, challenging circumstances.
